Bitwig • Sending MIDI Note On & Off to another software via OSC

Hello,

Does anybody have ideas on how I can broadcast Note On & Note Off from a Bitwig MIDI clip to another software using OSC ?

I would ideally liked to have the OSC messages be in the format "TrackName MidiNode MidiVelocity"
